PCNs have the potential to benefit patients by offering improved access and extending the range of services available to them, and by helping to integrate primary care with wider health and community services.
The benefits for patients include:
- better, more personalised health and care services, delivered more conveniently in settings closer to home
- support for individuals with more complex conditions that is better coordinated across different health and care services
- stronger support for patients to play a much greater role in making safe and informed decisions about their own health and care, taking full advantage of all local resources.
We decided to set up this PCN website to help us engage with our patients and residents as a PCN, showcase the work of our PCN, and make our PCN services easily accessible to you.
