Welcome to NeoHealth Primary Care Network (PCN)

PCNs have the potential to benefit patients by offering improved access and extending the range of services available to them, and by helping to integrate primary care with wider health and community services.

The benefits for patients include:

  • better, more personalised health and care services, delivered more conveniently in settings closer to home
  • support for individuals with more complex conditions that is better coordinated across different health and care services
  • stronger support for patients to play a much greater role in making safe and informed decisions about their own health and care, taking full advantage of all local resources.

We decided to set up this PCN website to help us engage with our patients and residents as a PCN, showcase the work of our PCN, and make our PCN services easily accessible to you.

Improving Access to Care – What we are doing for you in 2025/26

We are implementing several initiatives in 2025/26 to enhance patient access to care. Key improvements include reducing telephone wait times, promoting digital tools like the NHS App, and offering flexible appointment options including telephone, face-to-face, and online consultations. The PCN will also strengthen support for vulnerable patients and encourage the use of online services for greater patient control, aiming for faster responses to online consultation requests.
